North Fork Physical Therapy, PC is a private practice owned and operated by physical therapists. We take pride in providing consistent, quality care for our patients. The Difference Experience
Scott Czujko and David Kuck have earned doctoral degrees in physical therapy and have more than 20 years of clinical experience. They are Orthopedic Clinical Specialists, certified by the American Board of Physical Therapy Specialties. They also hold several certifications and credentials in other specialty areas including McKenzie, the hand, strength and conditioning, and golf.
Staff
Scott and Dave are the only full-time physical therapists at North Fork Physical Therapy and own and operate the practice. We have a small team of service-oriented individuals that are highly experienced to handle your treatment from start to finish. You will not be shuffled between therapists, and you will never be ignored. You will receive consistent treatments from the same team of therapists from your 1st visit until you are discharged.
Communication
At North Fork Physical Therapy you are the most important member of the health care team. It is very important to us that you understand the nature of your condition and your treatment options. We always welcome your input and are happy to answer any questions you may have regarding your treatment. We will also communicate with your physician to ensure continuity of care and keep all parties involved and up to speed.
